Notice is hereby given that the City of Glendale, Arizona, will receive proposals for furnishing the following:
RFP 12-22 -	Adult Court-Ordered Screening and Treatment Services
Due:	February 10, 2012 AT 2:00 P.M. (LOCAL TIME)
	The City of Glendale invites sealed proposals from qualified firms or individuals for adult court-ordered screening/assessment and outpatient treatment programs for defendants and follow-up tracking of defendant’s compliance/non-compliance for recommended programs. Addendum One Addendum Two
The proposal document is available at the office of the Materials Manager, 5850 W Glendale Avenue, Glendale, AZ, 85301, Glendale, Arizona, or can be downloaded from this web site. Each quote shall be in accordance with the specifications and instructions contained therein. Each proposal shall be sealed, identified and received on or before the due date and time listed above. Proposals shall be delivered to the Engineering Department’s front counter located on the third floor of City Hall, 5850 W Glendale Avenue, Glendale, AZ, 85301. The City of Glendale reserves the right to accept or reject any or all proposals and to waive informalities.
